Perth County has approved an 18 month pilot program which see a full time intensive case manager from Stratford Social Services join the County’s Paramedic Service Mobile Integrated Health Team.
The goal will be aimed to provide better health and social supports for the county’s most vulnerable.
The manager will work with a group of paramedics who provide care, support and practical solutions to vulnerable people in the community with in home visits and connecting them with doctors and other health care partners.
The entire cost of the project is covered from the province.
