The Listowel-Wingham Hospitals Alliance is working alongside the Huron-Perth Healthcare Alliance’s Mental Health Crisis Outreach Team and the Nurse Practitioner-led Primary Care outreach program.
The collaboration is to help high risk populations provide mental health support who do not have primary care physicians in the region to have access to an outpatient social worker.
The partnership will help provide social services and medical service for those needing support.
Details on the program are still being worked on but the Listowel-Wingham Hospitals Alliance are set to announce them soon.
