The Huron-Perth and Area Ontario Health Team is now up and running.
As one of the first Ontario Health Teams approved in the province, HPA-OHT strives to provide integrate care and delivery and funding to enable patients, families, communities, providers and system leaders to work together and build what is best in the province’s health care system.
In their first year of operation in Huron-Perth the 3 main goals intend to be Mental Health and Addictions, Infection Prevention and Control as well as Congestive Heart Failure.
